Wisconsin Chronic Disease Program

Chronic Renal Disease Program

To be eligible for the state funded Chronic Renal Disease Program (CRD), participants must be Wisconsin residents Medical Picture diagnosed as having end-stage renal disease. In addition, participants that are eligible for Medicare must pay Medicare Part B premiums in order to receive Wisconsin Chronic Disease Program (WCDP) renal disease benefits. If the participant is eligible for Medicare but chose to delay Medicare participation, the participant is not eligible to receive WCDP renal disease benefits until the participant begins paying Medicare Part B premiums.

If a CRD participant is a Medicare participant at the time of application, the eligibility date is determined by the date of the initial treatment. However, if the participant is Medicare-eligible but has chosen not to pay for premiums, the WCDP eligibility date is designated as the date that Medicare benefits are initiated.

Chronic Renal Disease participants are eligible to receive the following WCDP covered services:

  • Inpatient and outpatient dialysis and transplant treatments.
  • One pre-transplant dental examination, and X-rays.
  • Kidney donor transplant-related medical services.
  • Certain prescription medications.
  • Certain home supplies.
  • Certain laboratory and X-ray services.

Please reference the drug formulary regularly to determine drug coverage and appropriate copay amounts.

The state seeks repayment of CRD benefits provided to participants under the Estate Recovery Program.
