Access Request.
In order for primary contract agencies to enter data into the Substance
Abuse Prevention Services Information System (SAP-SIS), they must first register their agency and users with the
Bureau of Prevention, Treatment and Recovery (BPTR). By
registering, the primary contract agency is providing BPTR with
information about the agency and subcontract agencies as well as
authorizing users to access agency records. Some of the information
provided in the Agency/User Web Access Request will be used to pre-populate
fields in SAP-SIS records. Please, be as detailed as possible
when completing the request form. Detailed instructions for completing the
request are included on page three of the request form.
Prior to completing the Agency/User Web Access Request all users must have
their own Web Access Management System (WAMS) ID. If a user does not
already have a WAMS ID, they should go to https://on.wisconsin.gov/WAMS/SelfRegController
and complete the self-registration process. The WAMS ID registers
individual users to do secure business with the State of Wisconsin over
the Internet.
After completing the Agency/User Web Access Request, submit the request
to BPTR via

It may take up to 4-5 business days to process your request. You will
receive an e-mail confirming the receipt of your request. Once your
request has been processed you will receive an e-mail authorizing the
contract agency and its users to begin using the SAP-SIS reporting system.
Upon receiving authorization users may access the system at anytime to enter required data.
