A Flexible, Living Document to Help Address State Health
Plan Priorities
The Implementation Plan is designed to be flexible so it can be adapted
to fit the unique needs of neighborhoods, local communities, regions, and
the state population as a whole. The plan offers a rich set of opportunities
to promote collective action by community groups, professionals, agencies,
and organizations throughout Wisconsin. It is designed to galvanize
collective action to achieve the overarching goals of the state health plan:
(1) promote and protect health for all, (2) eliminate health disparities,
and (3) transform Wisconsin's public health system.
The Implementation Plan revolves around specific outcome objectives
(logic models and templates) to achieve the 16 priorities established in the
state health plan. The eleven health priorities influence both health and
illness; each priority has behavioral, environmental, and societal
dimensions. The five system priorities address public health infrastructure
improvements that will support the health priorities. Each priority must be
addressed in the context of all the others.
The Implementation Plan is not static and will be adapted and updated
over time by the Department in consultation with its partners. Such changes
will assure the plan fits the current context of public health in Wisconsin
and keeps pace with changes in knowledge, science, evidence, and practice.
