Approved CBRF Training Programs

Wisconsin Assisted Living Consumer Information

Contact the Regional Offices for information about the following:

  • Complaints about a CBRF

  • Starting a CBRF

  • The compliance history of a CBRF

Filing a Complaint Resource Sheet (consumer information)

All fires in a licensed health or residential care facility in Wisconsin must be reported to the Department of Health  Services within 72 hours (check specific rule requirements).

You may use the Fire Report Form (PDF, 23 KB) and fax it to the appropriate Regional Office or to (608) 267-7119 or call David Soens at (608) 261-5993. See also BQA memo 00-017. You may also E-mail the Division of Quality Assurance.
