The Bureau of Quality Assurance (BQA) is announcing a change to two mailing addresses
that will be used for the receipt of general mail. The two BQA offices affected are the
Madison Central Office and our Western Regional Office. The Madison Central Office change
is effective May 1, 1999 and the Western Regional Office change is effective July 19,
1999. Please make these permanent changes to your records.
Madison Central Office new address is
(new address effective May 1, 1999)
Western Regional Office new address is
(new address effective July 19, 1999)
Other BQA post office boxes used strictly for receipt of license/
certification fees or
mailings for the Caregiver Registry Investigation Unit, Nurse Aide Directory are still in
effect and unchanged.
