The purpose of this memo is to inform nursing homes that 42 CFR 483.75(l)(5)
(exit DHFS) requires
the nursing facility to keep a copy of the PASARR Level I, the Level II, and the results
of other preadmission screening(s) in the residents clinical record. Also, 42 CFR
483.106(b)(4)(ii) requires, in cases of transfer of a resident with mental illness or
mental retardation from a nursing facility to a hospital or to another nursing facility,
the transferring nursing facility is responsible for ensuring that copies of the
residents most recent PASARR and resident assessment reports accompany the
transferring resident.
