Wisconsin Emergency Assistance Volunteer Registry (WEAVR)

The Wisconsin Division of Public
Health, through the Public Health Preparedness and Response
to Bioterrorism Program, has implemented an on-line volunteer
registration system for health care and behavioral health
professionals. The Wisconsin Emergency Assistance Volunteer Registry,
or WEAVR, collects and stores contact |

information on volunteers interested in filling critical
response and recovery roles following a major public health emergency.
Based on the information collected from each volunteer, public health
officials are able to identify those individuals willing
to fill the specific
volunteer roles needed in
an emergency. Public health officials will use the WEAVR registry to generate
a list of volunteers who will be contacted based on the
information they provided. Those contacted will be given information on where to report and the
role that is needed, as well as the option to accept or decline the
opportunity. Training will be provided at the reporting site.
It is important to remember that:
- WEAVR membership and volunteer activities are in addition to any
professional or volunteer commitments you may have already established.
- You are the only person able to change the contact information
provided. Please make any changes to the data fields in the WEAVR
system as soon as they occur.

Registration Information
The WEAVR system and its associated processes are owned and managed
by the Wisconsin Division of Public Health. WEAVR is a secure,
password-protected, Web-based application that requires all users to
have a Wisconsin User ID. The information will be kept confidential
except for official use in an emergency event. If you already have a Wisconsin User ID, and
use the Wisconsin Health Alert Network and would like
to register as a volunteer, go to https://www.han.wisc.edu/
and select the "log in" tab to enter the HAN. Once you are logged onto
the HAN with your user id and password, go to "My HAN" option.
From that page, you will find an option to update your WEAVR
participation information--it should appear as: "Wisconsin
Emergency Assistance Volunteer Registry--Update your WEAVR participation
information."
If you do not have a Wisconsin User ID, please complete the following
2 step process.
Step 1
WEAVR is accessible via the Wisconsin Health Alert Network
(HAN). If you have never used the HAN before, and in order to log
on to WEAVR, you must have an active Wisconsin User ID. Visit the Web Access Management System
to Obtain
your Wisconsin User ID (Account). After you have obtained your Wisconsin User ID, return to this page
and go to step 2.
Step 2
To register with WEAVR you will need to use your Wisconsin User ID and password to
log-on to the Wisconsin
Health Alert Network (on that page, select the "log-in"
tab). Once you have signed on to the Health Alert Network, complete the registration
process which will include the option of participating in WEAVR. During
registration for the Health Alert Network, when prompted to select an organization,
if there is no organization that best matches your situation, please select
"WEAVR - Wisconsin Emergency Assistance Volunteer Registry" as
your organization.
