Detail Form Reports
A detail report contains all the boxes for a particular form. You may print a
copy of the contents of any form. To view a detail report:
- Click Reports on the menu bar.
- Click Detail Forms.
- Select the desired form type.
- To narrow a report request, enter the batch number from the communication
log file or the client’s Medicaid identification number.

- In the Form Status box, choose a status from the drop-down list or enter
the submission date in the Submit Date box.
- Click OK.
- Click Print to create a hard copy of the form.
Note: If no criteria is specified before clicking OK, all of the
forms in the database for that form type will be included in the report.
