Prior Authorization
Services Requiring Prior Authorization
Limits on Authorized Services
Authorization is limited to 12 hours in each 24-hour period and 60 hours in a calendar week for any one nurse, as stated in HFS 107.12(2)(b), Wis. Admin. Code. These limitations include all services for all recipients who are receiving Medicaid-covered services from the nurse. Wisconsin Medicaid will not approve a PA request for two consecutive 12-hour periods.
A 24-hour period should not be confused with a calendar day. For the purposes of Wisconsin Medicaid PDN services:
- Each calendar day is a 24-hour period that begins at midnight and ends at midnight.
- A calendar week begins with Sunday, ends with Saturday, and consists of seven consecutive calendar days.
