Coordination Services for Recipients Not Ventilator-Dependent

Coordination Services Documentation

The designated RN shall document all coordination services. Documentation in the recipient’s medical record is to include the extent and scope of specific care coordination provided (HFS 107.12(3)(c), Wis. Admin. Code).

Other information that must be included in the documentation is as follows:

• The type of services completed.
• The date and time of the services.
• The signature and title of the RN providing coordination services.

Each POC and PA/RF submitted by a nurse must include the name and license number of the coordinating RN.
