Coordination Services for Ventilator-Dependent Recipients

Documenting Coordination Services

The designated RN should document all coordination services. Documentation in the recipient’s medical record is to include the extent and scope of specific care coordination provided.

Other information that must be included in the documentation is as follows:

• The type of services completed.
• The date and time of the services.
• The signature and title of the RN providing coordination services.

The coordinating RN’s name and license number must be documented on both the Prior Authorization Request Form (PA/RF), HCF 11018, and on the recipient’s POC.
