Federal law requires that all persons requesting Medicaid or BadgerCare benefits and declaring to be U.S. citizens must show proof of their U.S. citizenship and identity in order to receive or continue to receive these health care benefits.
Who must show citizenship and identity?
Any person applying for or getting Medicaid or BadgerCare must follow this new federal rule. You will have at least thirty days to give this proof to your local county/tribal human service agency. If you are now getting Medicaid or BadgerCare, you will need to give this proof to your worker at your next yearly review.
You will not lose your benefits, as long as you are trying to get the proof
you need.
You will only have to do this one time, as long as you are getting Medicaid
or BadgerCare. Once you have given the proof, the information will be kept on
file.
If you are now getting Medicare or Supplemental Security Income (SSI) Medicaid, you do not have to follow this new rule.
What can I use to prove citizenship and Identity?
Some documents may be used to show both citizenship and identity. They are:
- U.S. Passport
- Certificate of U.S. Citizenship
- Certificate of U.S. Naturalization
If you do not have any of these documents, you will need to supply two items, one for citizenship and one for identity. (One from
the two lists below.)
Items you can use to prove citizenship include:
- U.S. Birth Certificate
- U.S. State Department Report of Birth Abroad
- U.S. Citizen ID card
- Adoption papers showing U.S. birth
- U.S. Military Record of Service
- Hospital record of U.S. birth
- Life or health insurance record showing U.S. birth
- Nursing home admission papers showing US birth
Items you can use to prove identity include:
- State driver license
- ID card issued by federal, state or local government
- School ID with picture
- U.S. Military ID card or draft record
- U.S. Military dependent ID card
- Native American Tribal document
- For children under 16, a signed Statement of Identity (HCF
10154) form
You may send or bring original or photocopies of these items to your worker. If you
are doing your application by mail or online, send photocopies of these items. If you are
having trouble getting what you need, contact your worker for help.
