This All-Provider Handbook is issued to all providers who are Wisconsin Medicaid
certified. Wisconsin Medicaid is also known as Medical Assistance
(MA), the Medical Assistance Program (WMAP), Title XIX, or T19. This
handbook contains information that applies to fee-for-service
Medicaid providers. |

Physical Therapy, Occupational Therapy, and Speech
and Language Pathology Services Handbook |
This handbook is a guide to Wisconsin Medicaid for all
Medicaid-certified physical therapy (PT), occupational therapy (OT), and
speech and language pathology (SLP) providers. Providers should maintain
and refer to this handbook in conjunction with the All-Provider
Handbook. This handbook completely replaces the Physical Therapy
Handbook - Part P, Div. II (published in January 1997), the Occupational
Therapy Services Handbook - Part P, Div. I (published in March 1998),
the Speech and Language Pathology Handbook (published in July 2003). |

| Family Care Guide
|
The Family Care Guide offers important
information for Medicaid fee-for-service providers who make services
available to Family Care – Care Management Organization (CMO)
members. While Family Care has not been implemented statewide, the
Family Care Guide is being sent to all Medicaid fee-for-service
providers in selected counties because providers in counties that do
not yet offer Family Care may, on occasion, need to provide services
to CMO members. |
