The forms and publications listed below are required to register a property in the Lead-Free/Lead-Safe Registry, or provide information for the maintenance of the property once it has been registered.

Forms

Application for Registration of Lead-Free or Lead-Safe Property  (DPH 44011, PDF, 48k) 
Lead investigators performing a lead-free inspection or lead-safe investigation can use this form to apply for registration of a property.

Lead-Free Inspection Affidavit of Property Owner (DPH 44014, PDF, 11k) 
Lead Investigators performing a lead-free inspection must have this completed form or a clearance report on file.

Lead-Free / Lead-Safe Property Registry, Training Course, Class and Roster Database Access Application (DPH 44010, PDF,12k)  
Lead Investigators, contractors, and training providers requesting access to the Wisconsin Asbestos Lead Database Online must complete this application.

Request for Repairs (DPH 44018, PDF, 20k)  
This form is to be given to tenants by the property owner of a registered lead-safe property before repairs commence.

Visual Inspection of Registered Lead-Safe Property (DPH 44005, PDF, 11k) 
Property owners of a registered lead-safe property are to complete this form annually to maintain certification.

Living in Registered Lead-Safe Property (PPH 44042)
Important information for tenants in a registered lead-safe property.

"Protect Your Family From Lead in Your Home", in English, Spanish, & Vietnamese (PDF) (Exit DHS - Environmental Protection Agency)  Owners of pre-1978 housing are required to give this to tenants when entering into or renewing a rental agreement or before more than 2 square feet of lead-based paint is disturbed.

The Wisconsin Lead-Free/Lead-Safe Registry: When Participation is Mandatory (DPH 44044, PDF, 175k)
This brochure provides guidance to property owners who are required to obtain a certificate of lead-free or lead-safe status.
