Varicella, Tdap, and PCV Requirements

Notice to: Health Care Providers, Day Care Providers, and Schools

Varicella and Tdap vaccines required in schools,
and PCV required in day care centers.

Effective September 1, 2008, the Wisconsin Student Immunization Law (HFS 144) has been changed to include second dose of varicella, a single dose of Tdap, and 2-3 doses of PCV vaccines. It will be phased in as follows:

*Varicella (chickenpox) vaccine
*Tdap vaccine (adolescent tetanus, diptheria, and acellular pertussis)
*PCV (Pneumococcal Conjugate Vaccine)

To be compliant, the parent (or guardian) must provide to the school or day care center one of the following:

  1. The date of vaccination (mm/dd/yy); or
  2. An indication that the child has a history of chickenpox; or
  3. A waiver for health, religious or personal conviction reasons.
