    Added two new data fields: 
    Numeric value between 00 and 96

LIVING ARRANGEMENT AT ADMISSION (Field 16)
    01 Street, shelter, no fixed address, homeless
    02 Private residence or household; living alone or   
         with others
    03 Supported or semi-supervised residence
    04 Specialized facility with on site supervision

    Wording change for code 5 to Not in the labor
    force–homemaker

  Removed LTS code 7 from code FT = Family care
  transfer (only allowed with LTS codes 2,4)
  Added code ND = Nursing home diversion

COSTS (Field 33)
  Notes added: LTS codes 2, 3, and N will accept up to 
  $99,999.99 per month for SPCs 112/56 and 112/57 
  Costs not allowed for SPCs 503, 896, 897, 898,
