Survey Information
How was I selected for this mail survey?
All parents with children attending the Franklin Elementary School are
being asked to complete this survey. Completion of the mail survey is
voluntary. However, It is important that as many parents as possible
complete the survey in order to get a complete picture of the success of
this initiative.
What is involved in the pre-event survey?
Enclosed with the mailing you
should have received is a survey to be completed by a parent in the
household. The survey is a short questionnaire on your current habits and
knowledge regarding eating fruits and vegetables and being active. The
survey will take about 10 minutes to complete. The purpose of the survey is
to gather information about whether the initiative has been successful.
There will be a follow-up survey in about a year to evaluate any changes
that may have taken place. The person taking the initial survey should also
take the follow-up survey. All survey responses are confidential. There is a
unique identifier on your questionnaire that will tell us who has completed
the survey. This information is used so we do not mail you another survey.
Your input on the survey is very important and we would really appreciate
you taking the time to complete the survey and returning it in the prepaid
envelope.
Must a parent attend or participate in the planned activities?
Participation in any of the activities and use of the Footprints to
Health materials are completely voluntary. There will be a number of
activities that you can do on your own. However, we hope that some of
the group activities will be of interest to you and that you will want to be
involved.
