 Wisconsin Collaborative Diabetes
Quality Improvement Project

The Wisconsin Collaborative Diabetes Quality Improvement Project highlights an extraordinary level of cooperation among diverse, competitive health maintenance organizations to improve diabetes care in Wisconsin.  Collaboration is key to this project's success.  This collaborative model may serve as the springboard for the expansion to other statewide quality improvement initiatives.

Wisconsin Collaborative Diabetes Quality Improvement Project Reports

The Wisconsin Collaborative Diabetes Quality Improvement Project Reports are provided on this page in PDF format.  The files are relatively large, and therefore may take a long time to open, view, or print.  If you would like to request a paper copy of the reports, contact the Wisconsin Diabetes Prevention and Control Program by calling: (608) 261-6855.

An additional report by the Wisconsin Collaborative Diabetes Quality Improvement Project is the Compendium of Diabetes Quality Initiatives From Wisconsin's Health Maintenance Organizations Involved in the Collaborative Diabetes Quality Improvement Project:
