Wisconsin Diabetes Advisory Group (DAG)

The Wisconsin Diabetes Advisory Group (DAG), convened by the Wisconsin Diabetes Prevention and Control Program in 1997, provides the foundation for active partnerships across the state.  It includes representatives from over 80 key organizations involved in diabetes in Wisconsin.

If you would like more information about the Wisconsin Diabetes Advisory Group, please send an email to Leah Ludlum or call her at (608) 261-9422.

Wisconsin Diabetes Advisory Group Partners

The mission of the Diabetes Advisory Group is to reduce the economic, social, physical, and psychological impact of diabetes in Wisconsin by enhancing the quality of services.

In 2004, the Diabetes Advisory Group developed the Wisconsin Diabetes Strategic Plan.  This plan provides a framework for Wisconsin organizations to mobilize around a set of common goals affecting all areas of diabetes care and prevention.

Wisconsin Diabetes Strategic Plan

Each year, the Diabetes Advisory Group develops a one-year DAG Workplan (previously known as the Performance Improvement Plan [PIP]).  The DAG Workplan is a one-year workplan for the members of the Diabetes Advisory Group to address priority areas of diabetes care and prevention.
