Emergency Medical Technicians licensed or trained in other states may be eligible for licensure in Wisconsin. Wisconsin requires current National Registry certification for licensure. Documentation and materials needed to determine license eligibility are outlined below for each license level. Applications are available from the EMS Systems Section or online.

First Responder

  • A complete First Responder/EMT Certificate/License Application
  • A copy of both sides of your current CPR for the Professional certification card
  • A copy of your Certificate of Completion for the US DOT First Responder course
  • A copy of the Certificate of Completion for your most recent First Responder refresher course
  • A copy of the Certificate of Completion for an approved 4-hour Terrorism Awareness course
  • Verification of Licensure form completed by each state you have ever been licensed in
  • Applicants with current First Responder (or higher) National Registry certification must also attach a copy of their Registry card

Materials will be reviewed and applicants notified of eligibility. Eligible applicants without current First Responder or higher National Registry certification will need to pass the First Responder National Registry examination. You will need a letter of authorization for entry to the exam that will be sent to you upon proof of eligibility.

EMT-Basic

  • A complete First Responder/EMT Certificate/License Application
  • A copy of both sides of your current CPR for the Professional certification card
  • A copy of the Certificate of Completion for the EMT-Basic course
  • A copy of the Certificate of Completion for your most recent EMT-Basic refresher course
  • A copy of the Certificate of Completion for an approved 4-hour Terrorism Awareness course
  • Verification of Licensure form completed by each state you have ever been licensed in
  • Applicants with current EMT-basic (or higher) National Registry certification must also attach a copy of their Registry card

Materials will be reviewed and applicants notified of eligibility. Eligible applicants without current EMT-Basic or higher National Registry certification will need to pass the EMT-Basic National Registry examination. You will need a letter of authorization for entry to the exam that will be sent to you upon proof of eligibility.

EMT-Basic IV

  • A complete First Responder/EMT Certificate/License Application
  • A copy of both sides of your current CPR for the Professional certification card
  • A Certificate of Completion of EMT-Basic training
  • A Certificate of Completion of EMT-Intermediate 85 training including:
    • A syllabus/course outline including the name and location of the institution where training was received
    • Dates of training
    • Topic outline of all didactic and practical training
    • Number of hours completed broken down by classroom didactic training, in-hospital clinical experience and supervised field experience
  • A Certificate of Completion of your most recent refresher training
  • A copy of the Certificate of Completion for an approved 4-hour Terrorism Awareness course
  • Verification of Licensure form completed by each state you have ever been licensed in
  • Applicants with current EMT-Intermediate 85 (based on 1985 US DOT national EMT-I curriculum) or higher level National Registry certification must also attach a copy of their card

Materials will be reviewed and applicants notified of eligibility. Eligible applicants without current EMT-Intermediate 85 or higher National Registry certification will need to pass the EMT-Intermediate 85 National Registry examination or successfully complete the Wisconsin Basic IV course and pass the Wisconsin EMT-Basic IV written and practical exams. The EMT-Intermediate 85 exam is not administered in Wisconsin.

EMT-Intermediate

  • A complete First Responder/EMT Certificate/License Application
  • A copy of both sides of your current CPR for the Professional and ACLS certification cards
  • Certificate of Completion of EMT-Basic training
  • Certificate of Completion of EMT-Intermediate training (based on 1999 US DOT EMT-I national curriculum) including:
    • A syllabus/course outline including the name and location of the institution where training was received
    • Dates of training
    • Topic outline of all didactic and practical training
    • Number of hours completed broken down by classroom didactic training, in-hospital clinical experience and supervised field experience
  • Certificate of Completion of your most recent refresher training
  • A copy of the Certificate of Completion for an approved 4-hour Terrorism Awareness course
  • Verification of Licensure form completed by each state you have ever been licensed in
  • Applicants with current EMT-Intermediate 99 (based on 1999 US DOT EMT-I national curriculum) or higher National Registry certification must also attach a copy of their card

Materials will be reviewed and applicants notified of eligibility. Eligible applicants without current EMT-Intermediate 99 National Registry certification will need to pass the EMT-Intermediate 99 National Registry examination. You will need a letter of authorization for entry to the exam that will be sent to you upon proof of eligibility.

EMT-Paramedic

  • A complete First Responder/EMT Certificate/License Application
  • A copy of both sides of your current CPR for the Professional and ACLS certification cards
  • Documentation of successful completion of EMT-Paramedic training (1000 hours minimum), including the following:
    • A syllabus/course outline including the name and location of the institution where training was received
    • Dates of training
    • A topic outline of all didactic and practical training including the number of hours completed broken down by classroom didactic training, in-hospital clinical experience and supervised field experience
    • Summary of all competencies achieved during EMT-Paramedic training
  • If your EMT-Paramedic course was less than 1000 hours, you may also submit the following for consideration to meet the 1000 hour requirement:
    • Verification of EMT-Paramedic work experience
    • Certificates of Completion for EMT-Paramedic continuing education courses
  • A copy of the Certificate of Completion for an approved 4-hour Terrorism Awareness course
  • Verification of Licensure form completed by each state you have ever been licensed in
  • Applicants with current EMT-Paramedic (based on 1998 US DOT national EMT-P curriculum) National Registry certification must also attach a copy of their card

Materials will be reviewed and applicants notified of eligibility. Eligible applicants without current EMT-Paramedic National Registry certification will need to pass the EMT-Paramedic National Registry examination. You will need a letter of authorization for entry to the exam that will be sent to you upon proof of eligibility.
