A quality Emergency Medical Services system reduces both the human
suffering and economic loss to society from the premature death and disability resulting
from accidents or sudden illness. Wisconsin EMS is dedicated to developing and maintaining
that EMS delivery system, ensuring that a uniform quality and standard of pre-hospital
emergency medical care is available to everyone in the State of
Wisconsin.

2008-2010 License Renewals
Ambulance service providers
and first responder providers along with individual EMTs and first
responders must all complete the renewal process prior to June 30,
2008.
Provider renewal information
was mailed in early February.
Individual renewal information was mailed mid-February.
Providers and individuals should use the pre-printed
forms that were sent to them.
These forms contain individual information currently shown
in the EMSS database. Most
individuals will only need to verify current address information,
read and answer the criminal history and driver’s violation
questions and sign and submit the renewal form.
Training information has been pre-entered into the EMSS
database by the training centers as individuals completed initial
or refresher training through the past biennium.
We are not able to re-print
these forms. A blank
individual renewal form is available below. Make sure to include
your name, address and license number when using this form.
