Re: Posting Assisted Living Facility Directories on the Internet
We are pleased to announce the posting of directories for all
state-regulated assisted living facilities to the Internet via www.dhfs.state.wi.us/bqaconsumer/AssistedLiving/AsLivDirs.htm.
Assisted living facility types include adult day care programs, adult
family homes, community based residential facilities, and residential
care apartment complexes.
This site contains a number of different directory formats:
- PDF documents readable using Acrobat Reader
- Microsoft Excel spreadsheets for sorting data in different ways
and for use as a source for creating labels
- All-state lists (sorted in order by county, then by community)
- Individual county lists for each provider type via either a table
of links or a clickable map
Directories will be re-posted monthly for adult family homes and
CBRFs and quarterly for adult day care programs and RCACs. Information
for these directories comes from databases maintained by the Bureau of
Quality Assurance (BQA) staff. It is up to the management and staff of
individual providers to inform BQA of changes in writing. Letters should
be sent to appropriate regional office with this information.
Directories contain information about individual facilities such as
facility name, address and phone; licensee name, address and phone;
ownership type; resident capacity, including gender numbers; date of
initial license; high/low monthly price ranges; types of clients served;
and (for RCACs) meals served and costs.
Consumers, providers and agencies will find using these electronic
directories to be efficient and cost-effective. Information that used to
be available only via mail for a charge, or by visiting a regional
office, can now be viewed online at any time without additional cost. We
are proud to make this contribution to Wisconsin’s expanding
electronic government projects.
Effective May 1st, 2004, BQA will discontinue providing
paper copies of directories or labels for sale.
