Federal Title V MCH Services Block Grant 

Each July 15th, the Family Health Section is required to submit an application to the U.S. Department of Health and Human Services, Health Resources and Services Administration, Maternal and Child Health Bureau (MCHB).  The purpose is to monitor Wisconsin's Title V MCH Services Block Grant progress, goal attainment and proper expenditure of  funds.

Excerpts of the 2008 Application / 2006 Annual Report are available below.  This includes a summary detailing activities conducted by local public health departments and other agencies during 2006; current progress on the national and state performance measures; and a summary of fiscal expenditures.

You are invited to provide input that will be utilized in planning for and writing Wisconsin’s 2009 Application / 2007 Annual Report.  For more information, please visit the Public Input page.   

The MCHB has posted on their website, Title V Information System (TVIS), the 2008 applications / 2006 reports from all states.  Via the TVIS website you can access narrative reports and forms for the Wisconsin application/report, as well as compare how Wisconsin is doing in relation to other states. Needs Assessment

In 2005, the Title V Maternal and Child Health (MCH) and Children/Youth with Special Health Care Needs (CYSCHN) statewide needs assessment was conducted for the period 2006 - 2010.  

The federal funding agency, the U.S. Department of Health and Human Services, Health Resources and Services Administration, requires that a statewide MCH/CYSCHN needs assessment be conducted every five years for two main purposes: 1) in order to receive federal funds, and 2) to help states make the most appropriate program and policy decisions that promote the health of women, children, adolescents, CYSHCN and their families.
