Accessing a Dentist
Some Federally Qualified Health Care Clinics have dental
clinics on site. You will need to contact each site to verify that
they are currently providing dental services for new patients. When
reviewing the clinic sites, try to locate and contact one closest to the
area in which you live.

Hygiene Program Clinics
Many dentists in private practice accept Medical Assistance (MA), however,
some of them are not accepting new patients who are covered by MA.
Please contact dentists in your area to inquire about obtaining dental
services.
Urgent Care Dental Form
Wisconsin Medicaid Urgent Care Dental In-State Emergency Provider Data
Sheet HCF11013 (Rev. 01/04).
The Urgent Care Dental Provider Form is ONLY for
non-Medicaid certified providers. It is intended to allow
non-Medicaid certified dentists to provide limited dental care on an
urgent basis. Only those procedures listed on the form can be
performed. This form in no way obligates a dentist to accept
Medicaid patients on an emergency or continuing basis.
