How to get materials

Below, you will find links to helpful materials that can be printed directly from this site. 

You can order many materials by completing this form.  After completing the form, email it to: fm-hcf-phc@dhfs.state.wi.us for processing. Most orders are sent out within 3 days.

Here is a list of the materials that you can order related to BadgerCare Plus. You will need to include the publication number (in bold below) of the item you want when ordering.

Poster PHC 10166 (size 24x36"). Includes space for a tear-off information sheet.

Poster PHC 10174 (size 24x36"). Includes space for a tear-off information sheet.

Poster PHC 10175 (size 24x36"). Includes space for a tear-off information sheet.

Tear-off sheets PHC 10166A (size 4.5x3.5")  To be included on any of the above posters.
