To order forms, brochures and other informational materials, select items from
the publications
directory. (PDF, 35 KB) 
You have the choice of placing your order in electronic or handwritten format. The
DHS Forms Center will send your materials via UPS within 7-10
working days (often much faster).
Ordering Electronically
To order electronically, download
the order form. For fastest service, submit the form as an attachment
via e-mail to: dhfsfmdphwic@wisconsin.gov.
Ordering by e-mail is the fastest and most efficient way to obtain
publications. In addition, you will receive an e-mail confirming your
order and providing information about publications that are new or out of
stock.
Handwritten Orders
To submit a handwritten order via regular mail, use the DMT-25.
