Requesting Additional Proof of Identity
Providers may request additional proof of identity from a recipient if they suspect fraudulent use of a Medicaid identification card. If another form of identification is not available, providers can compare a persons signature with the signature on the back of the Medicaid identification card if it is signed. (Wisconsin Medicaid encourages adult recipients to sign the back of their cards; however, it is not mandatory for recipients to do so.)
Verifying recipient identity, as well as eligibility, can help providers detect instances of fraudulent identification card use.
