Remittance and Status (R/S) Messages
Week of 9/1/08
Attention End-Stage Renal Disease Providers:
Medicaid-initiated adjustments began processing August 29, 2008, for
end-stage renal disease claims that denied on R/S Reports dated May 25,
2008, through June 29, 2008. Some end-stage renal disease claims appearing
on those reports were denied in error.
Attention Dental Claim Billers:
Effective for paper claims received on and after September 2, 2008,
ForwardHealth will accept dental claims submitted only on the American
Dental Association (ADA) 2006 Claim Form. Dental claims submitted on the ADA
2000 and 2002 claim forms will be denied.
Attention Institutional and Professional Claim Billers:
Effective for paper claims received on and after September 15, 2008,
ForwardHealth will accept claims submitted only on the 1500 Health Insurance
Claim Form (dated 08/05) or the UB-04 claim form. Claims submitted on
previous versions of these claim forms will be denied.
