Medical Home
The 2007 Medical Home Summit: Small Steps, Big Changes
was held November 15th. This event was designed for providers and
families to learn more about the concepts of medical home and
coordinated care for children and youth with special health care needs.
The following presentations were captured via webcast:

Sheri Johnson, PhD, Wisconsin Division of Public
Health Administrator - provides an overview of medical home concepts
and current national and state efforts surrounding medical home
implementation.

Brenda Sulock, a parent of children with special
needs along with Lisa Zetley, MD, share what a Medical Home means to
Brenda and her family.

Keynote speaker: Carl Cooley MD, Medical Director,
Crotched Mountain Foundation, New Hampshire, discusses Medical Home
as quality improvement and shares implementation strategies at the
primary care practice and larger system levels including practice
level outcome data.

Building Medical Home Partnerships: Small Steps: Big
Changes (Wisconsin Medical Home Toolkit)
www.wimedicalhometoolkit.aap.org Building a Medical Home Partnership: A Wisconsin Toolkit is
a new Web-based resource for Wisconsin
health care providers. The Toolkit shares the insights of a
committed group of families, physicians and care coordinators who
have been involved in a continuous quality improvement process to create
family-centered care for children and youth with special health care
needs. The Toolkit is
a roadmap to the tested tools and practice strategies recommended by
Medical Home Teams both nationally and here in Wisconsin.

An Introductory Toolkit packet containing a CD-Rom with four of
